Does Vaping Increase the Risk of High Blood Pressure? New Study Results
A systematic review published in the American Journal of Preventive Cardiology has evaluated the cardiovascular impact of e-cigarettes. By analyzing data from more than 700,000 participants, researchers assessed whether exclusive vaping or dual-use habits lead to hypertension.
Hypertension is a primary preventable contributor to stroke, heart disease, and chronic kidney disease. While nicotine temporarily spikes blood pressure and heart rate, short-term physiological changes do not automatically mean habitual vaping causes chronic high blood pressure.
To clarify this relationship, the research team analyzed 10 population-based observational studies. They separated exclusive e-cigarette users from dual users—those who both smoke and vape—to avoid misleading conclusions.
Key Findings: Exclusive Vaping vs. Dual Use
The study distinguished between prevalent hypertension (already present during assessment) and incident hypertension (newly diagnosed during the follow-up period).

For exclusive e-cigarette users, the data did not show a clear, undeniable link to high blood pressure. Although the estimates pointed to a slight increase in risk, the statistical margins were too wide to draw a firm conclusion.
In contrast, the risks for dual users were much more pronounced. Those who continued to smoke combustible cigarettes while using e-cigarettes showed a clear, heightened risk of both existing and newly developed hypertension.
Why the Findings Require Cautious Interpretation
The researchers rated the overall certainty of the evidence as very low. Several confounding factors make it difficult to isolate vaping as a direct cause of high blood pressure.
First, many exclusive vapers are former smokers. Their cardiovascular systems may still carry residual damage from years of previous exposure to tobacco smoke.
Second, dual users often differ from non-users in other lifestyle areas. Variations in diet, alcohol consumption, socioeconomic background, and baseline health can heavily influence blood pressure readings.
Finally, the analyzed studies used different methods to identify hypertension, ranging from self-reports to electronic health records, introducing potential bias.
What This Means for Smokers and Vapers
The current data is insufficient to prove that exclusive vaping increases long-term hypertension risk. However, health experts warn that a lack of statistical proof does not mean e-cigarettes are entirely safe.
For individuals who currently smoke and vape, the study highlights a clear message: dual use does not protect your cardiovascular health. To reduce hypertension risks, users must transition away from combustible tobacco entirely rather than combining both habits.
Future research must track participants over longer periods, separate former smokers from lifetime non-smokers, and collect precise data on nicotine strengths and device types.
