Turkey Considers Health Tax on Tobacco and E-Cigarettes

Turkish authorities are reviewing a proposal to introduce a targeted health tax on tobacco products, heated tobacco, and electronic cigarettes. This measure aims to offset public healthcare spending associated with smoking-related illnesses by channeling sector-specific tax revenue directly into the national health budget.

Prof. Dr. Toker Ergüder, a member of the Green Crescent Scientific Board, stated that the preliminary discussions examine the fiscal model implemented by the Philippines in 2012. Under that framework, tobacco excise taxes rose, generating a 114% increase in tobacco tax revenue by 2013 and directing 85% of incremental funds directly into universal healthcare coverage.

Regulatory Scope and Pricing Uncertainties

The World Health Organization supports similar tax earmarking strategies, noting that allocating tobacco taxes toward public healthcare reduces consumption while mitigating state economic losses. Media comparisons have also referenced the 1998 US Master Settlement Agreement, where tobacco manufacturers agreed to pay states over $206 billion across 25 years to cover smoking-related public health costs.

As of September 2026, the Turkish Ministry of Health has not presented a final bill or official implementation timeline. Officials clarified that the policy does not bill individual patient treatments to tobacco companies; instead, it establishes an industry-wide fiscal obligation.
