Great American Smokeout at 50: Smoking Hits Historic Low
This Thursday, November 20, marks a significant milestone in public health history: the 50th anniversary of the Great American Smokeout. Initiated by the American Cancer Society (ACS), this annual event held on the third Thursday of November has evolved from a symbolic one-day quit attempt into a powerful nationwide movement. As the initiative celebrates half a century of encouraging Americans to commit to a smoke-free life, it also highlights a dramatic victory: cigarette smoking among U.S. adults has plummeted from approximately 42% in 1965 to a historic low of just 11% today.
The impact of this decline is profound. An ACS-led study estimates that between 1970 and 2022, nearly 3.9 million premature lung cancer deaths were averted, gaining about 76 million years of life for the population. The “quit ratio”—the percentage of people who have ever smoked but successfully quit—reached a record 62% in 2022.

However, the battle is far from over. Dr. Bruce Waldholtz of the ACS emphasizes that the mission remains “saving lives,” but the focus has expanded. While smoking rates have fallen, lung cancer remains the leading cause of cancer death in the U.S., accounting for about one in five cancer-related deaths. In 2023 alone, there were an estimated 238,300 new lung cancer cases and roughly 127,000 deaths. Dr. Waldholtz stresses the critical importance of lung cancer screening, noting that only 18.1% of eligible current or former smokers were up to date with screening in 2022. “We have a great opportunity to save further lives by actual screening,” he stated.
New challenges also loom. Dr. Waldholtz warns of emerging risks, particularly vaping among young people. He cautions that vaping devices contain numerous substances beyond nicotine and can increase risks for cancer, cardiac, and pulmonary diseases. As the Great American Smokeout enters its next half-century, the goal remains clear: supporting smokers in their quit journey, addressing disparities in cessation success among different communities, and pushing for a future where the smoking rate reaches zero.
